For each scenario below, choose the most appropriate answer from the list above. Each option may be used once, more than once or not at all.
1. A newborn baby girl is examined immediately following normal vaginal delivery. She is clearly distressed and has a widespread purpuric rash. Inspection of the eyes shows a clouding of the lens. Chest auscultation reveals a machinery like murmur.
2. A 10 year old boy sustains 60% burns in a house fire. On day 3 post admission his condition has deteriorated. He is tachycardic, hypotensive and his wounds have reopened and are discharging copious amounts of fluid. There appears to be inflammation underneath the healthy skin around the wounds. Wound culture grows gram negative bacilli which fluoresce in the culture medium.
3. A 32 year old French cheese maker attends paediatric A&E with her one week old son. She says that he has been lethargic for the last 24 hours and cannot tolerate the light. On examination he is Kernig’s positive.
4. A 3 year old girl from the local traveller community presents with her father complaining of a one month history of cough. Her father explains that when she coughs “it sounds like she’s going to die”, going on to mention that she coughs so hard that “all the blood vessels in her eyes pop”.
5. A 15 year old girl attends her GP with her new boyfriend. She describes a 5 day history of pain on urination and some discomfort in the lower abdomen. She also mentions that she has to empty her bladder more frequently. Urine is positive for leucocytes and nitrites.
